It is preferable to use CAPITAL LETTERS TO FILL THE FORM. Please fill the form in clear legible writing.
Existing Investors with the AMC: Fill in your folio no & proceed directly to investment & payment details. (Signatures Mandatory)
Applicant Information: Fill in the Name & Address with Contact No., email ID and date of birth for the First Applicant / Second Applicant / Third Applicant, whichever applicable. (Pincode Mandatory)
Nomination: If the nominee is minor then name & signature of the Guardian is necessary. Also, fill in the date of birth (DOB) of the minor.
PAN is mandatory for all investments irrespective of the mode of holding. Copy of PAN Card needs to be attached with the application form/transaction slip.
For mutual fund investments worth Rs 50,000/- and above, submission of the Know Your Client (KYC) form is mandatory under the Prevention of Money Laundering Act 2002 (PMLA). For details please click here
Investment Details: Please mention the Scheme Name with Plan / Option on the Form.
Please ensure that the investment amount is equal to or greater than the minimum amount of subscription for the scheme.
Bank Details: Please provide Account Details of the first applicant. This is required for sending redemption/dividend payments etc. (Mandatory requirement as per Sebi Guideline)

Payment Details: Investment Amount, Cheque No, Cheque Date, Bank Name, Branch Address should be mentioned in the application form and the acknowledgment slip.
Match the physical cheque number and bank details with that mentioned on the application form.
Check for the date/year of the cheque.
Separate cheques should be submitted for each scheme/plan/option.
All Cheques/ DD's should be payable locally at the centre where the application is deposited.
Write the first applicants name and the Application number behind the cheque.
Declaration of Signature: If there is a single applicant, then only his signature is required. If there are two or three applicant then signature of all applicants is compulsory. In case of HUF, stamp of the HUF is required along with the Signatures of the Karta on the Application form.

SIP payment can be through Cheque/ECS/Direct Debit. Check whether you can use ECS or Direct Debit with your bank for the concerned scheme. This information is available in the form itself. Else use Cheques.
Mention No. of Cheques and Cheque details on the form in the space provided for post-dated cheques (PDCs) / subsequent installment details.
Frequency - Select the frequency as Monthly or Quarterly.
SIP Date should be chosen on the form and cheques should carry the same date at the frequency chosen.
